Oath & Affirmation Client Checklist
Step 1:
Verify Identity
- Provide a valid government-issued photo ID (e.g., driver's license, passport).
- You will be asked to confirm you are the individual matching the I.D. document provided.
Step 2:
Review the Document
- Ensure you do not sign any document without the Notary Public as a witness to signing.
- Ensure you (deponent) understands and agrees to the document(s) being signed.
Step 3:
Choose your Preference:
Oath or Affirmation
- State which option works best for you: Swearing an oath on a holy book or affirming the truth of this document? (both are legal and serve the same purpose).
Step 4:
Oath or Affirmation upon Notaries direction
Examples of Oath / Affirmation:
- Oath (with optional holy book):
“Please raise your right hand. Do you swear that the contents of this affidavit as subscribed by you are true, so help you God?” - Affirmation (no book):
“Do you solemnly affirm that the contents of this affidavit as subscribed by you are true?”
Step 5:
Witness the Signature
- Make sure required document(s) are signed in the Presence of the Notary Public
Step 6:
Apply Seal and Jurat
- Ensure Notary Public Affixes their notary seal / stamp.
- Review Jurat for time, date and year for accuracy
Step 7:
Obtain a record of your Notarization
- Receipt of services provided
